ISSUING A NEW RESIDENCE CARD
- When changing the information on the residence card (if the information no longer corresponds to reality)
- In case of loss, theft, damage, or misuse of the residence card
- Apply within 5 working days
Issuing a new residence card in connection with a CHANGE OF ADDRESS
A third-country national with legal residence is required to
- report a change of address of residence to the foreign police within 5 working days of the change (submitting the form for notification of change of address to the foreign police in person or by registered mail)
- apply for a new residence card if the address on the permit does not correspond to the actual address
Submitting an application for a new residence card:
- in person at the foreign police department (according to your new address) after booking an appointment via the reservation system (in the reservation system, select Documents / Passports and then „Application for a residence permit“)
or
- electronically (if you still have a valid and activated residence card for electronic use)
Required documents:
- application form – completed in Slovak and signed by the applicant (the version of the application for “renewal of temporary residence” is sufficient)
- valid passport
- valid residence card (if you still have it)
- proof of accommodation (not older than 90 days)
- administrative fee for issuing a residence permit and its delivery within the Slovak Republic: 10 EUR (within 30 days) or 39 EUR (within 2 working days)
Proof of accommodation may take the following forms:
• affidavit of property ownership (if you own the property)
• a lease agreement with the owner or user of the property and documentation regarding the right to use the property, in the case of a lease agreement with the user of the property
• affidavit of a natural or legal person on the provision of accommodation and documentation concerning the eligibility to use the property, in the case of an affidavit of the user of the property
• confirmation from the accommodation facility (e.g., hotel, hostel)
Important information:
Signatures on affidavits and rental agreements must be notarized (except for affidavits signed at the Foreign Police).
Documents must be originals or notarized copies.
The Foreign Police does not return documents (only passports).
